The easiest way to pay is online. Simply click the appropriate “Make a Payment” button above.

Please Note: You must choose the correct payment button based on the date of the date of service of the claim you are paying.

For Service Dates on or after 10/01/25: (219)924-8178 & ask for billing       M-Fri 9am-4pm
